Ages 18-34
$250/month
For adults navigating early adulthood, career, and lifestyle changes who want reliable, ongoing medical support.
Ages 35-54
$350/month
For adults managing busy lives or emerging health concerns who value time, access, and a doctor they can trust.
Ages 55+
$450/month
For patients seeking more attention, deeper follow-up, and long-term support through every stage of health.
You can choose to pay monthly, quarterly, or annually. A six-month minimum commitment is kindly requested.
The five hundred dollar initiation fee is waived for previously established patients.
Membership Inclusions
Appointments in Office or by Telehealth
All scheduled visits with Dr. Dorothy are included in your membership, whether in person or by telehealth, for routine care, follow-ups, or new concerns.
Direct Communication
Members may contact Dr. Dorothy directly during office hours by phone, text, or secure message for routine updates, brief questions, or clinical follow-up between visits.
Care Coordination
Support is provided for ordering labs, imaging, and arranging referrals, with attention to continuity and clear communication throughout the process.
Medication Management
Your care includes the review, renewal, and adjustment of medications as needed, based on ongoing clinical evaluation and follow-up.
Administrative Requests
When appropriate, forms, letters, or documentation needed for work, school, or travel are prepared as part of your membership.
